Terms and Conditions 

Fees:

Initial assessment: The initial speech and language assessment is charged according the type of assessment and age of the child. It is the client’s responsibility to confirm the fee payable. The fee for the assessment includes the assessment and a report. Any additional assessment sessions required to complete the assessment will be charged at an hourly rate.

Fees
Fees for Sessions:

The fee per session is based on the type and duration of the treatment. Session times will be confirmed following the assessment. Treatment sessions includes both direct therapy and parent liaison following the therapy. The fee for any introductory session or meeting with parents, patients or teachers is charged at an hourly rate. Therapy material provided will be charged for separately. Any additional reports requested will be charged at a set rate.

Payment of Fees:

Parents/carers are directly responsible for payment of invoices except  when covered by BUPA UK.  We do not invoice any other insurance companies directly. Parents can submit invoices settled directly to their respective insurance companies for reimbursement.  If you have medical insurance to cover the assessment and/or treatment, ensure that you have a valid authorisation number specifically for Analou Sugar.

Cancellation Policy:

The practice operates a strict 24 hour cancellation policy. Failure to attend an appointment or late cancellation will result in full fee payable for the appointment booked.

All reports are sent via encrypted email. If you wish not to receive any reports and/or correspondence via email, please notify Analou Sugar at the time of your appointment.

This privacy policy sets out how Analou Sugar, speech and language therapist (Trading under company name Speech and Language Therapy Centre Ltd) uses and protects any information that you give us as a current or new patient.

 

Speech and Language Therapy Centre Ltd is committed to ensuring that your privacy is protected. Should we ask you to provide certain information by which you can be identified when registering, then you can be assured that it will only be used in accordance with this privacy statement.

 

Speech and Language Therapy Centre Ltd may change this policy from time to time by updating this page. You will be notified of this with a copy of our new policy document.

 

As an independent speech and language therapist, Analou will act as data processor and date controller jointly with her PA. This means they are responsible both for processing the data and controlling how and why the data is being collected.

 

What information Speech and Language Therapy Centre Ltd collect about me?

 

  • Information you provide by filling in registration forms, questionnaires or treatment consent forms, radiology or audiology reports.

  • Information you provide when you refer yourself or are referred by a doctor.

  • Clinic letters from other consultants involved in the care of your child.

 

How will Speech and Language Therapy Centre Ltd use the information they collect about me?

 

We require this information to understand your needs and provide you with a better service, and in particular for the following reasons:

  • To contact you about an invoice.

  • To contact you about a request / enquiry you have made with us, for example requesting an appointment with Analou Sugar.

  • To provide you with information you may have requested, for example a copy of a clinic letter.

 

Medical details you send to us electronically for assessment of your condition, arrangement of appointment or any other purpose may be shared with other medical professionals for the benefit of your care.

 

By sending us your details, health records etc. you consent and accept that it may be necessary for us to forward these clinical details electronically or in other formats.

 

Speech and Language Therapy Centre Ltd will never share your information with other organisations for marketing, market research or commercial purposes.
